Vaccination is not required to enter Peru.

Centers of Vaccination
If you have planned to carry out a trip to the amazon area or jungle of Peru, it is advisable to be vaccinated against the yellow fever.

In Lima
 
International Airport Jorge Chávez: It is located in the 2° floor, medical post in which you can be vaccinated before taking a flight to the jungle.
Open: 24 hours, seven days of the week.

Center of International Vaccination: It is located in the National Hospital "2 de Mayo".
Address: Parque de la Medicina w/n, Lima.
Open: Monday to Saturday from 07:30 to 13:30 hrs.

Center of Vaccination: It is located at one side of the "Hospital del Niño".
Address: Jirón Independencia No. 121, Breña
Open: Monday to Saturday from 08:00 to 12:30 hrs.

In the Provinces
It is also possible to be vaccinated at your arrival to the airports of Iquitos and Puerto Maldonado.

Note: If you are planing to speng a long time in the jungle part of Peru consult with your doctor, for you may want to be vaccinated against hepatitis, typhoid, tetanus and take an anti malaria treatment.
